We caught a local bus to Suphan Buri about three hours away from Kachanaburi and then a minibus to Ayuthaya which is about an hours drive north of Bangkok. Mandalay airport was quite far out of the city - it took about an hour to get there by taxi, we arrived at the very sparse international airport - probably one of the most laid back airports we have been, we couldn't even find any check in desks when we arrived!
